  1. Health Maintenance Organizations (HMOs):

PCPs serve as gatekeepers for accessing specialist care, and referrals are generally required for visits to specialists. HMOs offer comprehensive coverage for preventive care and medical services within the network, with lower out-of-pocket costs and no deductibles for in-network services.   2. Preferred Provider Organizations (PPOs):
